This is a screening tool used to assess the severity of depression symptoms in adults. It consist of 9 questions that cover the common symptoms of depression:
Low Mood
Loss of interest or pleasure
Changes in sleep
Fatigue
Poor appetite or overeating
Feeling worthless or guilty
Difficulty concentrating
Thoughts of death or suicide

Each question is scored on a scale of 0 to 3, with higher scores indicating greater severity of symptoms.
The total score ranges from 0 to 27, with higher scores indicating more severe depression.

0 - 4: Minimal
5 - 9: Mild
10 - 14: Moderate
15 - 19: Moderately Severe
20 - 27: Severe
What it measures
A self-administered questionnaire used to screen for and assess the severity of generalized anxiety disorder.
Consists of 7 questions about the frequency of anxiety over last 2 weeks
Each question is rate 0 (not at all) to 3 (nearly every day)
The total score ranges from 0 to 21, with higher scores indicating a greater severity of anxiety
A score of 8 or higher suggest a possible case of GAD
