Understanding resident risks is key to providing proactive, personalized care. Data For Care categorizes risk into two main types:
1. Compliance & Adherence
These risks focus on how well residents follow prescribed routines or participate in care activities. Examples include:
Medication Non-Adherence: Forgetting, refusing, or inconsistently taking medications, which may require supervision or tailored strategies to ensure safety.
Therapy Non-Participation: Declining or disengaging from therapy sessions or activities, affecting progress and overall well-being.
2. Mood & Affect
These risks relate to residents’ emotional and mental states, which can influence daily functioning and participation in care. Examples include:
Anxiety: Excessive worry, restlessness, or physical tension that interferes with engagement in therapy or activities.
Depression: Persistent low mood, feelings of hopelessness, or loss of interest that impacts motivation and daily life.
By scoring and tracking these risks, care teams can identify issues early, implement targeted interventions, and tailor care plans to each resident’s unique needs.
Takes into account the 2 assessment categories:
Compliance/Adherence: Measures how consistently a resident follows their care plan — including medication routines, therapy participation, and other prescribed activities. It helps identify where extra reminders, supervision, or support may be needed.
Mood/Affect: Assesses a resident’s emotional well-being and how mood influences daily functioning. It focuses on signs of anxiety, depression, or other emotional changes that may impact engagement and quality of life.
